PDE5 inhibitors (sildenafil, tadalafil, avanafil) are among the better-tolerated medicines in common use. The side effects are mostly mild, come from the same blood-vessel-widening action that makes them work, and tend to ease as your body settles. This article explains the common ones, how to manage them, and the rare effects that need prompt attention.
Knowing what to expect matters, because a side effect you were warned about is a minor nuisance, while the same effect unexpected can be enough to put you off a treatment that would otherwise have worked well. Almost everything here is manageable without stopping.
These affect a minority of men, are usually mild, and often improve with continued use:
Headache: the most common, from widened blood vessels. Management: stay hydrated, and consider a lower dose if it is troublesome.
Facial flushing: a warm, red feeling in the face and neck. Management: usually settles; avoid taking it with a lot of alcohol, which adds to it.
Blocked or runny nose: nasal lining vessels widen too. Management: usually mild and temporary.
Indigestion or reflux: more common with sildenafil. Management: avoid large fatty meals around dosing.
Back or muscle aches: more associated with tadalafil, typically 12 to 24 hours after a dose. Management: simple pain relief if needed; it passes.
Almost all of the common side effects come from the same action that makes the tablet work. A PDE5 inhibitor widens blood vessels, and it does not do this only in the penis. Widen the vessels in the head and you get a headache or flushing; widen those in the nose and you get congestion; relax the ring of muscle at the top of the stomach a little and you get reflux. Knowing this is reassuring: the side effects are a sign the medicine is doing what it should, not a sign of harm, and they tend to fade as your body adjusts.
If a side effect is bothering you, there are usually three levers before stopping altogether: lower the dose, switch to a different tablet in the same family, or, for tadalafil, move to a low daily dose which tends to be gentler. Many men who get headaches on one tablet are fine on another. Taking the tablet with plenty of water, and not stacking it with a heavy meal or a lot of alcohol, also helps. A few everyday things change how strong the tablet feels, which is worth understanding so a side effect does not catch you off guard:
Grapefruit juice: can slow how the body clears the tablet, raising levels and making side effects more likely. Keep it modest around dosing.
Alcohol: in larger amounts, adds to flushing, headache, and dizziness because it also lowers blood pressure.
Certain prescription medicines: some antifungals, antibiotics, and HIV treatments raise PDE5 inhibitor levels, so a lower dose may be advised.
None of these is dangerous in the way nitrates are. They are simply reasons the dose is chosen with your other medicines and habits in mind.
A small number of men notice mild, temporary visual changes with sildenafil, such as a blue tinge to vision or increased light sensitivity, because of a related enzyme in the retina. This is usually harmless and short-lived. Sudden loss of vision or hearing, however, is different: it is rare, but if it happens you should stop the medication and seek medical advice promptly.
Priapism is an erection that will not go down, lasting more than about four hours, and it is a medical emergency. Blood trapped in the penis for too long can damage the tissue permanently. It is rare with tablets, but if an erection lasts beyond four hours, do not wait it out: go to A&E. Treated early, it is usually resolved without lasting harm.
Most side effects are a nuisance rather than a warning. A few are a reason to stop and get advice the same day:
n erection lasting more than four hours (priapism).
udden loss of vision or hearing.
hest pain, fainting, or severe dizziness after a dose.
