On-demand vs daily tadalafil: which suits you

Tadalafil can be taken in two ways: a larger dose when you need it, or a small dose every day. Both are licensed, both are effective, and the choice comes down to how your sex life actually works rather than which is stronger. This article walks through the two rhythms so you can pick the one that fits.

It is worth saying at the outset that there is no wrong answer here, and nothing is permanent. The aim is simply to match the medicine to your life, and the right match is the one that you forget about because it just works in the background.

On-demand tadalafil

On-demand tadalafil means taking a single dose (commonly 10mg or 20mg) ahead of sex. Its advantage over shorter-acting tablets is the window: a dose lasts up to 36 hours, so you are not watching the clock. Take it earlier in the day or evening and you have a long, relaxed period in which sex can happen naturally.

It suits men who have sex less often, perhaps once or twice a week or less, and who are happy to take a tablet with some forward planning. Because the window is long, the planning is light.

Daily tadalafil

Daily tadalafil means a low dose (2.5mg or 5mg) taken at the same time every day. Instead of dosing for a single occasion, you keep a steady level in your body all the time, so you are ready whenever the moment arises. There is no planning and no association between taking a tablet and having sex.

It reaches its full, steady effect after about five to seven days, so it is worth giving it a week before judging it. It tends to suit men who have sex more than twice a week, or who simply prefer spontaneity and would rather not think about timing at all.

There is also a quieter benefit some men value: because the tablet is no longer tied to the act of sex, it removes a small but real source of performance anxiety. Nothing has to be planned, produced, or timed, which for some men is exactly the pressure that was getting in the way.

Spontaneity versus simplicity

The honest trade-off is this:

On-demand: you take a tablet only when you need it, which means less medication overall, but you do need to remember to take it ahead of time.

Daily: total spontaneity and nothing to remember in the moment, but you are taking a tablet every day whether or not you have sex.

Neither is better in principle. Men who value not planning usually prefer daily; men who have sex occasionally usually prefer on-demand.

A useful side effect of the daily dose

Low-dose daily tadalafil is also licensed to treat the urinary symptoms of an enlarged prostate (benign prostatic hyperplasia), such as a weak stream or needing to pass urine more often. If you have both ED and these symptoms, daily tadalafil can address the two together. The doctor will raise this if it is relevant to you.

How to choose, and how to switch

A simple way to decide is to count: if you are likely to want sex more than about twice a week, daily tends to make sense, because you are taking a tablet most days anyway and you gain complete spontaneity. If sex is less frequent or unpredictable, on-demand is usually the better fit, and you take far fewer tablets overall.

You are not locked in. Plenty of men start on-demand to see how they respond, then move to daily once they know the medicine works for them and they want to stop planning. Others go the other way as their circumstances change. A switch is a small adjustment, made at your assessment or at a later review, and it does not mean starting from scratch.

Side effects and safety are the same

Whichever rhythm you choose, the side-effect profile is the same, and tends to be milder on the lower daily dose. Headache, flushing, a blocked nose, indigestion, and back or muscle aches are the usual ones, and they often settle with use.
