The DVLA D4 medical for a lorry, bus or coach licence, completed and signed by a registered GP in a 30-minute appointment. In person, by video where the licence allows, or by phone.
Driver medicals
A doctor works through the DVLA Group 2 standard and completes your D4 form at the appointment. That means a vision check on the Snellen chart, a blood pressure reading, a urine dipstick for glucose and protein, and a look at your heart, chest, nervous system and general fitness to hold a professional licence. If you have a condition the DVLA takes an interest in, such as diabetes, a heart condition or a sleep disorder, the doctor records the detail the form asks for.
The D4 does not need your full GP records, though bringing a summary helps if your history is complex. If any finding falls short of the Group 2 standard, the doctor will tell you plainly and, where it helps, point you to DVLA medical enquiries rather than leave you guessing.
