The TfL TPH/204 medical for London taxi and private hire drivers, and TfL 204 or DVLA Group 2 renewals, completed by a registered GP in a 30-minute appointment.

What's involved
The examination is the DVLA Group 2 standard, the same as a D4: vision on the Snellen chart, blood pressure, a urine dipstick, and a check of your heart, chest, nervous system and general fitness. The doctor completes and signs your TPH/204 form at the appointment.
Good to know
TfL asks that the examining doctor has sight of your full medical record. It is not mandatory, but applications can be delayed without it, so bringing your GP summary from the NHS App is worth doing. If you cannot get it in time, the doctor can still assess you and will note on the form that the assessment was based on the history you gave.

What to bring
Bring photo ID, your glasses or lenses if you wear them, your current medicines, and your GP summary from the NHS App, which TfL prefers the doctor to see.
Pricing
£180 for a 30-minute appointment and certificate. Any bloods, ECG or drug screening are an additional charge, confirmed with you at the consultation before anything is sent. You pay after your consultation in person; video and phone consultations are paid at booking.
