Other tests: Two character references and a police certificate also required
The report: Completed on the destination's own form, an embassy template, or a covering report we draft, with all signatures and stamps included.

Guidance states the certificate may be 'from a doctor specified by the Immigration Division at Vaiola Hospital OR from overseas', implying an overseas doctor may be acceptable, but this was not confirmed on a primary government page (forms page unreachable).
