Ritalin XL is a once-daily methylphenidate capsule. Half the dose is released straight away and the other half about four hours later, so one morning capsule works like two doses of immediate-release methylphenidate taken four hours apart. Meflynate XL, Metyrol XL and Focusim XL are capsules of the same design. Modified-release methylphenidate brands are not interchangeable, so we supply the brand you are already taking.

What you need to know
ADHD in children (6+), young people and adults. The adult licence makes it a straightforward prescribing choice in adult ADHD when methylphenidate is preferred.
Methylphenidate increases dopamine and noradrenaline activity in the brain, which helps attention, focus and impulse control. Each capsule holds two kinds of beads: one releases straight away, the other about four hours later.
People with ADHD whose treatment was started by an ADHD specialist and who are already taking, or have been advised to take, Ritalin XL or one of the same-design capsules.
Order online and we review your request. A prescriber checks your consultation form for clinical appropriateness and contacts you if anything else is needed. Once approved, your medication is dispensed and delivered to your door. Controlled drugs require additional clinical checks. We may need to contact you or your GP before dispensing.

Very common: decreased appetite, insomnia, headache, dry mouth, nausea. Common: palpitations, tachycardia, weight loss, abdominal pain, mood changes (anxiety, irritability), tremor. Uncommon: psychotic symptoms (hallucinations, paranoia). Monitor: growth (in children), cardiovascular parameters, psychiatric symptoms.
Do not use in severe cardiovascular disease, uncontrolled hypertension, hyperthyroidism, phaeochromocytoma, during or within 14 days of MAOIs. Use with caution in anxiety, psychosis, tics, seizure disorders, substance misuse history.
MAOIs: contraindicated (hypertensive crisis). Antihypertensives: stimulants may reduce their effect. SSRIs: caution with serotonergic effects. Alcohol: avoid.
Limited data in pregnancy. Stimulants may affect fetal growth. Use only if benefit clearly outweighs risk. Discuss with specialist before conception. Not recommended during breastfeeding.
Take once a day in the morning. Swallow whole, with or without food. If you cannot swallow capsules, they can be opened and the beads sprinkled on a spoonful of apple sauce or yoghurt and swallowed straight away without chewing. Taking it later in the day can make sleep harder. Modified-release methylphenidate brands release the medicine differently, so tell us which brand you take and do not switch brands without speaking to a doctor.

Driving & machinery
May affect ability to drive. Dizziness, drowsiness, and visual disturbances have been reported. Do not drive or operate machinery until you know how it affects you.
Age restriction
Methylphenidate/lisdexamfetamine: children 6 and over, and adults. Atomoxetine: children 6 and over, and adults. Guanfacine: children 6-17 (MR formulation). All must be initiated by a specialist in ADHD.
Storage
Store below 25°C in a dry place. Keep out of reach and sight of children. Do not use after the expiry date.
