In brief…
These are once-daily methylphenidate tablets. About 22 per cent of the dose releases straight away and the rest steadily over 10 to 12 hours, so one morning tablet covers early morning to early evening. Concerta XL is the original brand. Delmosart is made to release the same way, and it is the one we dispense if you are starting on this type of tablet. If you already take Concerta XL, choose it below and we will keep you on it. Modified-release methylphenidate is prescribed and dispensed by brand, so your prescription and your box will name the same one. If you take Ritalin XL, which is a capsule that releases half the dose straight away and half about four hours later, it has its own page.

Have you been assessed for ADHD?

What you need to know
ADHD in children (6+), young people and adults as part of a comprehensive treatment programme, when a long-acting methylphenidate preparation is needed for all-day symptom coverage.
Same active drug as IR methylphenidate (dopamine and noradrenaline reuptake inhibition), released slowly through the day. Concerta XL does this with an OROS osmotic pump, and Delmosart is made to give the same release over the day. In Concerta XL the tablet has a drug-coated outer layer (immediate release) surrounding an osmotic core that pushes drug out through a laser-drilled hole at a controlled rate over 10-12 hours. The tablet shell passes intact in the stool - this is normal.
Patients who need all-day coverage without midday dosing. Ideal for school/work environments. The 22:78 release profile suits patients who need steady afternoon and early-evening coverage. Tablets must be swallowed whole - they cannot be crushed, chewed or split (this would destroy the OROS mechanism and release the entire dose at once). Not suitable for patients who can’t swallow tablets whole.

Very common: decreased appetite, insomnia, headache, dry mouth, nausea. Common: palpitations, tachycardia, weight loss, abdominal pain, mood changes (anxiety, irritability), tremor. Uncommon: psychotic symptoms (hallucinations, paranoia). Monitor: growth (in children), cardiovascular parameters, psychiatric symptoms.
Do not use in severe cardiovascular disease, uncontrolled hypertension, hyperthyroidism, phaeochromocytoma, during or within 14 days of MAOIs. Use with caution in anxiety, psychosis, tics, seizure disorders, substance misuse history.
MAOIs: contraindicated (hypertensive crisis). Antihypertensives: stimulants may reduce their effect. SSRIs: caution with serotonergic effects. Alcohol: avoid.
Limited data in pregnancy. Stimulants may affect fetal growth. Use only if benefit clearly outweighs risk. Discuss with specialist before conception. Not recommended during breastfeeding.
Take once daily in the morning with breakfast. MUST be swallowed whole - never crush, chew or split. The ghost tablet shell appears in the stool - this is completely normal and doesn’t mean the medication hasn’t worked. Coverage typically lasts 10-12 hours. Some patients find coverage wears off in the late afternoon and need an IR top-up for evening activities or homework. MHRA warning: do not assume different methylphenidate MR brands are interchangeable - they have different release profiles. Switching brands requires specialist oversight.

Driving & machinery
May affect ability to drive. Dizziness, drowsiness, and visual disturbances have been reported. Do not drive or operate machinery until you know how it affects you.
Age restriction
Methylphenidate/lisdexamfetamine: children 6 and over, and adults. Atomoxetine: children 6 and over, and adults. Guanfacine: children 6-17 (MR formulation). All must be initiated by a specialist in ADHD.
Storage
Store below 25°C in a dry place. Keep out of reach and sight of children. Do not use after the expiry date.
