Like GTN, isosorbide mononitrate releases nitric oxide, widening blood vessels and improving blood flow to the heart. The modified-release version provides consistent protection throughout the day.

Adults with stable angina who need preventive treatment beyond beta-blockers or calcium channel blockers. Not suitable with PDE5 inhibitors (ED medications).

Modified-release tablets: take once daily in the morning. Standard tablets: the dosing schedule creates a 'nitrate-free' period to prevent tolerance. Headache is very common initially but usually improves. Dizziness and flushing are possible.
