Type 2 diabetes (with or without other agents), heart failure (with reduced or preserved ejection fraction), and chronic kidney disease - regardless of diabetes status.

Dapagliflozin blocks SGLT2 transporters in the kidneys, which normally reabsorb glucose back into the blood. By blocking them, excess glucose is excreted in the urine. This lowers blood sugar, blood pressure, and body weight. The kidney and heart benefits appear to go beyond the glucose-lowering effect.

Adults with type 2 diabetes (especially with cardiovascular disease or risk factors), heart failure, or CKD. Not for type 1 diabetes. Requires adequate kidney function to lower glucose (but still protects kidneys at lower eGFR). Avoid in recurrent urinary or genital infections.

Common: urinary tract infections, genital thrush (candidiasis), increased urination, dizziness. Uncommon: dehydration, hypotension. Rare but serious: diabetic ketoacidosis (can occur even with normal blood glucose - seek help for nausea, vomiting, abdominal pain, excessive thirst, difficulty breathing). Fournier's gangrene (very rare).

Diabetic ketoacidosis. Use with caution in recurrent UTIs/genital infections, volume depletion, elderly patients.

Diuretics: increased risk of dehydration and hypotension. Insulin/sulfonylureas: increased hypoglycaemia risk - dose reduction may be needed.

Oral antidiabetics are generally not recommended in pregnancy - insulin is preferred for blood glucose control. Metformin is sometimes used in pregnancy under specialist supervision (especially for gestational diabetes and PCOS). Stop other oral agents before conception.

Take once daily in the morning. Increases risk of genital thrush (especially in women) and urinary tract infections - good hygiene helps. Can cause dehydration, so drink plenty. Rare but serious: diabetic ketoacidosis (DKA) can occur even with normal blood sugar - seek urgent help if you develop nausea, vomiting, abdominal pain and rapid breathing. Promotes modest weight loss (2-3kg typically).
