Empagliflozin

Type 2 diabetes, heart failure (with reduced ejection fraction), and cardiovascular risk reduction.

Same mechanism as dapagliflozin - blocks SGLT2 in the kidneys to excrete excess glucose in urine. Lowers blood sugar, blood pressure, and weight. The cardiovascular benefits are thought to involve improved cardiac energy metabolism and reduced cardiac workload.

Adults with type 2 diabetes (especially with established cardiovascular disease) and adults with heart failure. Same cautions as dapagliflozin regarding genital infections and DKA.

Common: urinary tract infections, genital thrush (candidiasis), increased urination, dizziness. Uncommon: dehydration, hypotension. Rare but serious: diabetic ketoacidosis (can occur even with normal blood glucose - seek help for nausea, vomiting, abdominal pain, excessive thirst, difficulty breathing). Fournier's gangrene (very rare).

Diabetic ketoacidosis. Use with caution in recurrent UTIs/genital infections, volume depletion, elderly patients.

Diuretics: increased risk of dehydration and hypotension. Insulin/sulfonylureas: increased hypoglycaemia risk - dose reduction may be needed.

Oral antidiabetics are generally not recommended in pregnancy - insulin is preferred for blood glucose control. Metformin is sometimes used in pregnancy under specialist supervision (especially for gestational diabetes and PCOS). Stop other oral agents before conception.

Take once daily in the morning. Same side effect profile as dapagliflozin: genital thrush, UTIs, dehydration risk, rare DKA. The choice between empagliflozin and dapagliflozin often comes down to specific indications and local prescribing preferences. Both are excellent medications.
