Utrogestan (Micronised Progesterone 100mg)

Endometrial protection alongside oestrogen HRT in women who still have a womb. Also helps with sleep (has a mild sedative effect). Can be used cyclically (12-14 days per month) or continuously.

Progesterone opposes the stimulating effect of oestrogen on the womb lining (endometrium). Without it, oestrogen alone can cause the lining to thicken abnormally, increasing the risk of endometrial hyperplasia and cancer. Micronised progesterone is well-absorbed and has a favourable side effect profile compared to synthetic progestogens.

Women with a womb who are taking systemic oestrogen HRT. Cyclical use (12-14 days per cycle) for perimenopausal women, continuous use for postmenopausal women (12+ months since last period). Contains peanut oil - not suitable for peanut allergy.

Common: breast tenderness, headache, nausea, fluid retention, mood changes, irregular bleeding (usually settles within 3-6 months). Uncommon: bloating, leg cramps. Rare: venous thromboembolism (DVT/PE), stroke. Transdermal preparations carry lower VTE risk than oral.

Known or suspected breast cancer, undiagnosed vaginal bleeding, active or recent VTE, untreated endometrial hyperplasia, active liver disease, porphyria.

CYP3A4 inducers (carbamazepine, phenytoin, rifampicin, St John's Wort) reduce effectiveness. Thyroid replacement: may need dose adjustment.

Not for use in pregnancy or breastfeeding. Vaginal oestrogen (very low systemic absorption): may be used during breastfeeding under medical advice.

Take at bedtime (the mild sedative effect actually helps sleep). Swallow whole - don’t chew. Can also be used vaginally (off-licence but widely recommended) for better womb protection and fewer side effects. Cyclical use will cause a monthly withdrawal bleed. Contains peanut oil. Side effects: bloating, breast tenderness, mood changes, drowsiness.
