Nausea and vomiting, particularly in Parkinson’s disease (drug-induced nausea from dopamine agonists) and gastroparesis.

Domperidone blocks dopamine receptors peripherally in the gut (prokinetic effect) and at the chemoreceptor trigger zone (anti-emetic effect). Because it doesn’t readily cross the blood-brain barrier, it doesn’t worsen Parkinson’s symptoms or cause central movement disorders.

Adults under 60kg should use lower doses. Particularly useful in Parkinson’s disease. Restricted use due to cardiac risk - lowest effective dose for shortest duration. Not for children (withdrawn for paediatric use in UK).

Metoclopramide: drowsiness, restlessness, extrapyramidal reactions (especially in young adults). Domperidone: headache, dry mouth. Ondansetron: headache, constipation, warmth/flushing. Cyclizine: drowsiness, dry mouth, blurred vision. Prochlorperazine: drowsiness, extrapyramidal effects.

Metoclopramide: GI obstruction/perforation/haemorrhage, phaeochromocytoma, epilepsy. Domperidone: severe hepatic impairment, prolonged QT, concomitant QT-prolonging drugs. Ondansetron: congenital long QT syndrome.

QT-prolonging drugs: risk with domperidone, ondansetron. Metoclopramide: enhances effects of some drugs (increased absorption), opposes dopamine agonists. Cyclizine: enhanced sedation with alcohol/CNS depressants.

Cyclizine and prochlorperazine: commonly used for morning sickness. Metoclopramide: 2nd line for hyperemesis. Ondansetron: used in severe cases (some concern about cardiac malformations - discuss with specialist). Domperidone: limited data in pregnancy.

Maximum 10mg three times daily for up to 7 days. MHRA restrictions: increased risk of serious cardiac arrhythmias and sudden cardiac death, particularly at higher doses, in older patients, and with other QT-prolonging medications. ECG may be recommended before starting. Avoid in severe liver disease. Take 15-30 minutes before meals.
