Hyoscine butylbromide blocks muscarinic receptors on smooth muscle in the gut, reducing spasm. It doesn’t cross the blood-brain barrier well, so it causes fewer central nervous system side effects than other anticholinergics. However, it does have some drying effects (dry mouth, reduced sweating).

Adults and children over 6. Avoid in glaucoma, myasthenia gravis, and paralytic ileus. Caution in prostate enlargement (can worsen urinary retention). Don’t use if you have unexplained abdominal pain that hasn’t been investigated.

Metoclopramide: drowsiness, restlessness, extrapyramidal reactions (especially in young adults). Domperidone: headache, dry mouth. Ondansetron: headache, constipation, warmth/flushing. Cyclizine: drowsiness, dry mouth, blurred vision. Prochlorperazine: drowsiness, extrapyramidal effects.

Metoclopramide: GI obstruction/perforation/haemorrhage, phaeochromocytoma, epilepsy. Domperidone: severe hepatic impairment, prolonged QT, concomitant QT-prolonging drugs. Ondansetron: congenital long QT syndrome.

QT-prolonging drugs: risk with domperidone, ondansetron. Metoclopramide: enhances effects of some drugs (increased absorption), opposes dopamine agonists. Cyclizine: enhanced sedation with alcohol/CNS depressants.

Cyclizine and prochlorperazine: commonly used for morning sickness. Metoclopramide: 2nd line for hyperemesis. Ondansetron: used in severe cases (some concern about cardiac malformations - discuss with specialist). Domperidone: limited data in pregnancy.

Take 1-2 tablets up to three times daily. Works best if taken at the onset of cramps. Can cause dry mouth and constipation. Don’t use regularly without medical advice if you’re over 40 and symptoms are new - get investigated first.
