Nausea and vomiting, gastroparesis (diabetic and other causes), and as an adjunct in migraine (to improve absorption of pain relief).

Metoclopramide blocks dopamine D2 receptors centrally (anti-emetic effect) and peripherally in the gut (prokinetic effect). It increases gastric emptying, tightens the lower oesophageal sphincter, and coordinates upper GI motility. This dual action makes it unique among anti-emetics.

Adults under 20 (restricted to short-term use - max 5 days - due to risk of movement disorders). Not for children or young adults under 20 (higher risk of dystonic reactions). Caution in epilepsy and Parkinson’s disease.

Metoclopramide: drowsiness, restlessness, extrapyramidal reactions (especially in young adults). Domperidone: headache, dry mouth. Ondansetron: headache, constipation, warmth/flushing. Cyclizine: drowsiness, dry mouth, blurred vision. Prochlorperazine: drowsiness, extrapyramidal effects.

Metoclopramide: GI obstruction/perforation/haemorrhage, phaeochromocytoma, epilepsy. Domperidone: severe hepatic impairment, prolonged QT, concomitant QT-prolonging drugs. Ondansetron: congenital long QT syndrome.

QT-prolonging drugs: risk with domperidone, ondansetron. Metoclopramide: enhances effects of some drugs (increased absorption), opposes dopamine agonists. Cyclizine: enhanced sedation with alcohol/CNS depressants.

Cyclizine and prochlorperazine: commonly used for morning sickness. Metoclopramide: 2nd line for hyperemesis. Ondansetron: used in severe cases (some concern about cardiac malformations - discuss with specialist). Domperidone: limited data in pregnancy.

Maximum 5 days’ use (MHRA restriction). Take 30 minutes before meals. Can cause drowsiness, restlessness, and dystonic reactions (involuntary muscle movements - more common in young adults). Long-term use risks tardive dyskinesia. Not suitable under 20 years. Avoid in Parkinson’s disease, pheochromocytoma, or bowel obstruction.
