Overactive bladder (urgency, frequency, incontinence), detrusor instability, and off-licence: hyperhidrosis (excessive sweating).

Oxybutynin blocks muscarinic receptors on the bladder’s detrusor muscle, reducing involuntary contractions. For hyperhidrosis, it blocks the muscarinic receptors on sweat glands, reducing sweat production. The patch delivers the drug through the skin, avoiding first-pass liver metabolism that creates the metabolite responsible for most side effects.

Adults with overactive bladder. The patch formulation is better tolerated. For hyperhidrosis: increasingly prescribed off-licence for people with debilitating sweating. Significant anticholinergic burden with oral form - avoid in elderly patients if possible.

Antimuscarinics (oxybutynin, solifenacin): dry mouth (very common), constipation, blurred vision, drowsiness, cognitive impairment (especially in elderly). Mirabegron: hypertension, UTI, headache. Tamsulosin: dizziness, postural hypotension, retrograde ejaculation.

Antimuscarinics: urinary retention, severe GI conditions, uncontrolled narrow-angle glaucoma, myasthenia gravis. Mirabegron: severe uncontrolled hypertension. Tamsulosin: history of orthostatic hypotension.

Antimuscarinics: enhanced anticholinergic effects with other anticholinergic drugs. Mirabegron: increases digoxin levels. Tamsulosin: enhanced hypotension with alpha-blockers and PDE5 inhibitors.

Limited data. Use only if benefit outweighs risk.

Oral: 2.5-5mg two to three times daily. Patch: apply twice weekly. Dry mouth, constipation and drowsiness are common with oral form (less with patch). Not recommended for elderly patients (anticholinergic cognitive effects). For hyperhidrosis: typically lower oral doses. The patch is generally the preferred formulation if oxybutynin is chosen.
