Nausea and vomiting from various causes, vertigo, labyrinthitis, Ménière’s disease, and migraine-associated nausea.

Prochlorperazine blocks dopamine D2 receptors in the chemoreceptor trigger zone and also has anti-cholinergic and anti-histaminic actions in the vestibular system, which is why it’s particularly effective for vertigo and balance-related nausea.

Adults. Buccastem M available OTC for adults over 18. More sedating than ondansetron. Avoid in Parkinson’s disease, severe liver disease, and elderly patients with dementia.

Metoclopramide: drowsiness, restlessness, extrapyramidal reactions (especially in young adults). Domperidone: headache, dry mouth. Ondansetron: headache, constipation, warmth/flushing. Cyclizine: drowsiness, dry mouth, blurred vision. Prochlorperazine: drowsiness, extrapyramidal effects.

Metoclopramide: GI obstruction/perforation/haemorrhage, phaeochromocytoma, epilepsy. Domperidone: severe hepatic impairment, prolonged QT, concomitant QT-prolonging drugs. Ondansetron: congenital long QT syndrome.

QT-prolonging drugs: risk with domperidone, ondansetron. Metoclopramide: enhances effects of some drugs (increased absorption), opposes dopamine agonists. Cyclizine: enhanced sedation with alcohol/CNS depressants.

Cyclizine and prochlorperazine: commonly used for morning sickness. Metoclopramide: 2nd line for hyperemesis. Ondansetron: used in severe cases (some concern about cardiac malformations - discuss with specialist). Domperidone: limited data in pregnancy.

Buccal tablet: place between upper lip and gum, let dissolve (don’t chew). Oral tablet: 5-10mg up to three times daily. Causes drowsiness. Can cause akathisia (restlessness), dystonia and other movement disorders, especially in young people. Avoid in Parkinson’s. Short-term use recommended.
