Lower urinary tract symptoms (LUTS) associated with BPH: weak stream, frequency, urgency, hesitancy, nocturia, and incomplete emptying.

Tamsulosin selectively blocks alpha-1A adrenergic receptors, which are concentrated in the prostate, bladder neck and urethra. Blocking these receptors relaxes smooth muscle in these areas, reducing the squeeze on the urethra and improving urine flow. It’s more selective than older alpha-blockers, causing less blood pressure drop.

Men with LUTS due to BPH. Works within days for symptom relief. Can be combined with finasteride for larger prostates. Caution with blood pressure medications. Can cause ‘floppy iris syndrome’ during cataract surgery - tell your ophthalmologist.

Antimuscarinics (oxybutynin, solifenacin): dry mouth (very common), constipation, blurred vision, drowsiness, cognitive impairment (especially in elderly). Mirabegron: hypertension, UTI, headache. Tamsulosin: dizziness, postural hypotension, retrograde ejaculation.

Antimuscarinics: urinary retention, severe GI conditions, uncontrolled narrow-angle glaucoma, myasthenia gravis. Mirabegron: severe uncontrolled hypertension. Tamsulosin: history of orthostatic hypotension.

Antimuscarinics: enhanced anticholinergic effects with other anticholinergic drugs. Mirabegron: increases digoxin levels. Tamsulosin: enhanced hypotension with alpha-blockers and PDE5 inhibitors.

Take once daily after the same meal each day. Can cause dizziness on standing (postural hypotension), especially the first few doses - stand up slowly. Abnormal ejaculation (retrograde - reduced or absent ejaculate) occurs in about 10% of men. IMPORTANT: tell your eye surgeon you take tamsulosin before any eye surgery.
