Fungal nail infections (onychomycosis), ringworm (tinea corporis), athlete’s foot (tinea pedis) not responding to topical treatment, and fungal scalp infections.

Terbinafine inhibits squalene epoxidase, an enzyme in the early stages of ergosterol synthesis. This causes toxic squalene to accumulate inside the fungal cell while depleting ergosterol, effectively killing the fungus (fungicidal rather than just fungistatic).

Adults with confirmed fungal infection (ideally nail clippings sent for culture before starting). Treatment is long: 6 weeks for fingernails, 3 months for toenails. Liver function monitoring recommended. Not usually first-line for children.

Take once daily with food. Treatment duration: 6 weeks (fingernails), 3 months (toenails), 2-4 weeks (skin infections). The nail won’t look better immediately - you have to wait for the healthy nail to grow out (6-12 months for toenails). Liver function tests before and during treatment. Can cause taste disturbance, GI upset and rarely severe liver injury or skin reactions. Report any persistent nausea, jaundice or rash immediately.
