Co-codamol 30/500 (Codeine 30mg / Paracetamol 500mg)

Moderate pain not adequately controlled by paracetamol or NSAIDs alone. Used for acute pain from injury, dental procedures, post-surgical pain, and some chronic pain conditions.

Paracetamol works centrally to reduce pain perception. Codeine is converted to morphine in the liver, which then activates opioid receptors to block pain signals. The combination attacks pain through two different mechanisms, which is more effective than either drug alone.

Adults with moderate pain. Not suitable for children under 12. Use with caution in people over 65, those with respiratory problems, or anyone with a history of substance misuse. About 8% of people lack the enzyme to convert codeine to morphine, so it simply won't work for them.

Common: constipation (very common - consider laxative prophylaxis), nausea, vomiting, drowsiness, dizziness. Uncommon: dry mouth, sweating, itching, confusion. Serious: respiratory depression (especially in overdose or with other CNS depressants), dependence and withdrawal symptoms with prolonged use.

Do not use in acute respiratory depression, severe COPD exacerbation, paralytic ileus, acute abdomen. Codeine: do not use in children under 12, or under 18 after tonsillectomy/adenoidectomy. Not suitable for ultra-rapid CYP2D6 metabolisers (codeine).

Benzodiazepines and other CNS depressants: risk of profound sedation, respiratory depression, coma, and death. Alcohol: enhanced sedation - do not drink alcohol. MAOIs: risk of serotonin syndrome (tramadol specifically). SSRIs with tramadol: serotonin syndrome risk.

Avoid in pregnancy if possible, especially near delivery (risk of neonatal respiratory depression and withdrawal). Codeine is contraindicated in breastfeeding (risk of morphine toxicity in infant). Other opioids: present in breast milk - use only under medical supervision.

Take 1--2 tablets every 4--6 hours, maximum 8 tablets in 24 hours. Contains paracetamol -- don't take other paracetamol products alongside it. Can cause drowsiness, constipation and nausea. Risk of dependence with use beyond a few days. Not for long-term use without regular review.
