Diclofenac is a strong inhibitor of COX enzymes, with a slightly different selectivity profile to ibuprofen. This gives it potent anti-inflammatory and pain-relieving properties, particularly for joint and musculoskeletal conditions.

Adults with inflammatory pain conditions. Because of cardiovascular concerns, it's generally prescribed at the lowest effective dose for the shortest possible time. Not suitable for people with established cardiovascular disease, uncontrolled high blood pressure, or active GI bleeding.

Common: gastrointestinal disturbances (dyspepsia, nausea, diarrhoea), headache. Uncommon: dizziness, oedema, raised blood pressure. Serious: GI bleeding/ulceration (risk increases with age, dose, and duration), cardiovascular events (heart attack, stroke) with long-term use, renal impairment.

Do not use if you have active GI bleeding or peptic ulceration, severe heart failure, severe renal or hepatic impairment, history of GI bleeding with previous NSAID use. Avoid in the 3rd trimester of pregnancy. Celecoxib: avoid in sulfonamide allergy.

Anticoagulants (warfarin, DOACs): increased bleeding risk. ACE inhibitors/ARBs: reduced blood pressure effect and increased renal risk. Lithium: increased levels. SSRIs: increased GI bleeding risk. Other NSAIDs: do not take more than one NSAID at a time.

Avoid in the 3rd trimester (risk of premature closure of the ductus arteriosus and delayed labour). Use in 1st and 2nd trimester only if clearly needed and at lowest dose for shortest duration. Present in breast milk in small amounts - short courses of ibuprofen are generally considered compatible.

Take with food. Higher cardiovascular risk than ibuprofen or naproxen, so used cautiously. Stomach protection usually co-prescribed. Not available over the counter as tablets in the UK (gel only). Keep courses as short as possible.
