Seasonal allergic rhinitis (hayfever) at 120mg, and chronic idiopathic urticaria (hives) at 180mg.

Fexofenadine is an active metabolite of terfenadine (an older antihistamine that was withdrawn). It blocks H1 receptors without crossing the blood-brain barrier, providing potent, truly non-sedating antihistamine activity. Unlike some antihistamines, it isn’t metabolised by the liver, so fewer drug interactions.

Adults and children over 12 with allergies not adequately controlled by OTC antihistamines. Non-sedating. Fewer drug interactions than most antihistamines. Dose adjustment in kidney impairment.

Generally well tolerated. Uncommon: headache, drowsiness (less common with newer antihistamines), dry mouth, fatigue.

Hypersensitivity. Use with caution in epilepsy, prostatic hypertrophy, urinary retention, narrow-angle glaucoma (sedating antihistamines).

Minimal interactions with non-sedating antihistamines. Avoid grapefruit juice with fexofenadine.

Non-sedating antihistamines (cetirizine, loratadine): limited data but widely used without evidence of harm. Generally preferred over sedating antihistamines in pregnancy. Compatible with breastfeeding in short courses.

Take once daily. Avoid taking with fruit juice (grapefruit, orange, apple) as it can reduce absorption. Truly non-sedating. One of the safest antihistamines in terms of cardiac effects. Can be used long-term.
