Asthma not adequately controlled on inhaled corticosteroids alone. Also used in COPD with repeated exacerbations.

Fluticasone reduces airway inflammation (preventer action). Salmeterol is a long-acting beta-2 agonist (LABA) that keeps airways relaxed for 12 hours. Together, they address both the inflammation and the bronchoconstriction that drive asthma symptoms.

Adults and children (over 4) with asthma needing step-up from preventer alone. Also adults with COPD who have frequent exacerbations despite bronchodilator therapy. Available as Accuhaler (dry powder) or Evohaler (MDI).

SABA (salbutamol): tremor, headache, palpitations, muscle cramps. ICS (beclometasone, fluticasone, budesonide): oral thrush (candidiasis), hoarseness - rinse mouth after each use. LABA (formoterol, salmeterol): tremor, palpitations, headache. High-dose ICS: bruising, adrenal suppression.

LABA: must not be used without an ICS in asthma (monotherapy increases risk of asthma death). Tiotropium: hypersensitivity to atropine derivatives.

Beta-blockers: may reduce effectiveness of salbutamol and other beta-agonists - avoid non-selective beta-blockers in asthma. Potassium-depleting diuretics: increased risk of hypokalaemia with high-dose beta-agonists.

Salbutamol and ICS: can be used in pregnancy - uncontrolled asthma is more dangerous than the medications. Do not stop preventer inhalers during pregnancy. Compatible with breastfeeding.

Use twice daily, every day. NOT for acute symptom relief - keep your reliever inhaler handy. Rinse mouth after use. Side effects include hoarseness, oral thrush, headache and muscle cramps. Salmeterol must always be used with a corticosteroid in asthma - never on its own.
