
Most people notice a change by week four to six. Full extinction usually takes three to four months. Here is the honest timeline.
People almost always want to know how soon they will feel a difference. The honest answer is that it is gradual. The method works by extinction, a slow unlearning, so the timeline is set by how often you drink and how reliably you take the tablet. It is a fade, not a switch.
Many people build up gently, from a quarter tablet for the first several days to a half, then more. This keeps side effects (mild nausea is the most common) to a minimum while your body adjusts. You will not feel a dramatic change in your drinking yet; the medicine is just reaching the right dose, and the real work has barely started.
Once you are on the full dose and taking it reliably an hour before each drink, the extinction begins. You may notice drinks feel a little less satisfying, or that you do not reach for the second quite so automatically. It is subtle. Most people cannot quite put their finger on it; they just notice they drank a bit less than they meant to.
By the end of the first month most people describe a clearer reduction. The number of drinks per session is often the first to fall: three glasses of wine instead of five, two pints instead of four. The frequency of drinking tends to drop a little later.
This is also where some people feel discouraged, because the change is not more dramatic yet. That is normal, and sticking with the tablet through this stretch is the strongest predictor of success.
Across the second and third months most people see a substantial drop. Many find their drinking has roughly halved; some are down to a glass or two at weekends. A few discover they no longer want to drink at all. The pattern varies from person to person.
This is the point where the brain's learned reward has faded a long way. Cravings are quiet. Drinking, when it happens, feels much less like the pull it used to be, more something you choose than something that tugs at you.
What happens next is yours to decide. Many keep taking it to hold the extinction, then carry a few tablets for the occasions that still tempt them, one before drinking, nothing otherwise. Some stay on it long term. Some find alcohol no longer does anything for them and stop. Repeats are there whenever you need them, with no fixed course and no pressure either way.
Some people see change sooner, drinking halved by week four. Others take six months to see a substantial shift. Roughly one in five do not respond meaningfully and need a different approach.
